First Visits
Please bring your medical records, immunization history, test results, procedures, and a list of current medications on your first visit to the Center. Please also bring your insurance information, if applicable.
Insurance Accepted
We accept Medicare, Medicaid*, Aetna, Horizon, Healthnet, Tri Care, and QualCare health plans.
*NOTE: Currently the only Medicaid HMO we participate in is University Health Plan (UHP). We recommend that you check with your insurance carrier to ensure that services you receive through the Family Medicine Center will be covered.
Emergency Care
We recognize that emergency situations arise and the Family Medicine Center makes every effort to address your needs promptly. If an emergency arises after business hours, our answering service will forward the message to the physician on call who will advise you on how to proceed. In some circumstances, the CentraState Emergency Department may be used.
Prescription Refills and Specialist Referrals
All requests for prescription refills and/or referrals should be made during the Family Medicine Center's regularly scheduled office hours. Please contact the Family Medicine Center at least one week before your refill runs out.
Telephone Call
We encourage you to call with questions concerning your treatment. If you need to speak with a physician and one is not immediately available, your call will be returned as soon as possible.
Monitoring System Helps with Teaching
A closed-circuit television monitoring system at the Family Medicine Center allows the faculty physicians to directly observe patient visits for the purposes of teaching and training. You may request that cameras be disconnected if observation will make you uncomfortable. If you agree to have your visit videotaped, you will be asked for written permission. If you request that cameras be disconnected during your visit, it will in no way adversely affect the treatment that you receive at the Family Medicine Center.
