No appointment is required for most tests. Appointments are necessary for glucose tolerance tests, lactose tolerance tests and semen analysis, and should be made through the Centralized Scheduling department at (732) 294-2778.
Before receiving service from the Outpatient Lab, you will need to register for your lab work in the Outpatient Registration department, which is located next to the Outpatient Lab. You will need to bring your prescription, insurance information and personal identification, such as a driver’s license. If you do not have insurance or if services from CentraState’s Lab are not covered by your insurer, you will be billed for your lab work.
Pre-registration is available for Lab services by calling (732) 294-2681. Patients who have registered as a recurring patient having the same procedure on a repeated basis may report directly to the Outpatient Lab. The recurring registration must be renewed in the Outpatient Registration department every 90 days. Prescriptions are valid for one year.
