CentraState’s Outpatient Lab at Monroe

If you’re looking for quality laboratory testing, you can now look closer to home. Our highly-skilled phlebotomists at CentraState’s outpatient lab are careful, considerate and experienced – providing quick and compassionate service while preventing or limiting potential discomfort. We offer comprehensive diagnostic testing, performing more than 500 different diagnostic procedures.

Laboratory services include:

• Blood count tests
• Urinalysis tests
• Glucose testing
• CBC (Complete Blood Count)
• Mono tests
• Thyroid testing
• BMP (Basic Metabolic Panel)
• PSA (Prostate Specific Antigen)
• Lipid profile
• Transfusion medicine (Blood Bank)

Convenient Location and Hours

• CentraState’s Outpatient Lab is located CentraState’s Medical Office, 312 Applegarth Rd., Suite 103.
• Open Monday through Friday, 7 a.m.–3 p.m. Wednesday, 7 a.m. – 6 p.m.
• No appointment necessary (except for glucose tolerance testing)
• Services are provided for all ages – infants through senior adults

Insurance
We participate in most insurance plans. If you are unsure of the coverage provided by your plan for laboratory services, call your insurer to confirm.

Lab Test Results
Patient reports are automatically faxed to all physicians who accept faxed reports. Reports are mailed to physicians who prefer this service. You will need to contact your physician for your results.

Physicians at the satellite offices are private practitioners and not employees or agents of CentraState Medical Center. These physicians rent offices from CentraState and have privileges to admit patients to, or perform procedures at, CentraState Medical Center.
 
Physicians who have privileges at CentraState must meet certain educational and experience requirements; however, CentraState is not responsible for the specific care provided to you by your physician.
 
CentraState cannot assure that all physicians or consultants participate with all insurance plans. If you have services performed at CentraState, your physician’s bills will be separate from the Medical Center bill. Please contact your insurance company to find out if your physician is covered by your insurance plan. If you have questions regarding a physician bill, please contact your physician’s billing office.
