Risk Assessment and Diagnostic Evaluations

Individualized Risk Assessment

Prior to your appointment, you will be asked to complete an individualized cardiac assessment to identify your risk factors for heart disease. This will help us to better understand your lifestyle habits.

Questions will include:
• Family health history
• Physical activity level
• Nutrition habits
• Ability to handle stress
• Tobacco and alcohol usage

Diagnostic Evaluations

After reviewing your history and assessment results, you will receive a physical examination and we will perform several screening tests, whichmay include:

• Blood pressure check
• Complex cholesterol panel
• Blood glucose testing
• Weight/body mass index
• Echocardiography
• Nuclear imaging
• Cardiac catheterization

Based on your assessment and testing results, we will work closely with you and your primary care physician to develop a customized wellness plan and address any risk factors identified
