Arriving prepared will ease the check-in process when you arrive for your appointment. Please print and fill out the consent form and the pre-vaccination checklist questionnaire, and bring with you to the walk-in event.

Vaccination Event FAQs

The vaccine center is located in the Star and Barry Ambulatory Campus, 901 West Main Street, Freehold, NJ 07728. Once inside, proceed to the Health Awareness Center. Click here for directions.

Please print and fill out the consent form and the pre-vaccination checklist questionnaire and bring with you to the walk-in event. Please be prepared to bring relevant forms of ID, such as a driver’s license or employer-issued identification, and insurance card, if applicable. No one will be turned away without ID or insurance. Note: You will not receive a bill and this will not impact your deductible.

Park in the Blue Parking Zone closest to the Fitness Center and enter via Entrance #6, the main entrance to the Star and Barry Tobias Ambulatory Campus. You can access a printable map for vaccine parking information here.

The longest wait times tend to be at the start of each walk-in event. For the shortest wait times, arrive after 9 am for Tuesday events or after 4 pm for Thursday events.

CentraState cannot honor requests for specific vaccines. The Pfizer two-dose regimen is used for our walk-in events.

After checking in, you will be directed to the vaccination area where you will receive your vaccine and a vaccination card. You will then be directed to a waiting area where you will be monitored by a licensed clinician for 15 minutes. If you have health concerns about receiving the vaccine, please discuss them with your own primary care provider prior to receiving the vaccine.

Yes, you will receive a vaccine card at your first appointment that includes information on the vaccine type, the date each dose was administered, and the location the vaccine was given. Make sure you keep it for future reference. If you lose your vaccine card, you can request a copy of your COVID-19 vaccine record from the New Jersey Immunization Information System.

The date of your second dose appointment will be set exactly three weeks from your first dose. It will be confirmed with you upon receiving your first dose at the walk-in event. Please plan to arrive for your second dose at the same time you arrived for your first dose.

Yes. If you received your first dose of the Pfizer vaccine elsewhere you can receive your second dose of Pfizer at CentraState’s walk-in clinic. You will need to print and fill out the pre-vaccination checklist and COVID-19 vaccine consent form above and bring your CDC vaccination card as evidence of your first vaccine. The Pfizer vaccine is the only vaccine available for second doses.
