CentraState Healthcare System is pleased to announce that we will be entering into a strategic collaboration with Horizon Blue Cross Blue Shield of New Jersey to improve quality of care, increase value and enhance the patient experience.
In 2019, CentraState will implement Horizon’s successful “Episodes of Care” programs, starting with Orthopedics and Maternity/Newborn care. By integrating Horizon’s patient-centered, value-based care models into our existing programs, we will strengthen our efforts to provide patients with the best possible clinical care and overall experience.
CentraState remains a full provider in the Horizon Network and continues to accept all Horizon members, including OMNIA patients at a TIER 2 level of benefit. Additionally, all doctors in CentraState Healthcare Partners (our Physician Hospital Organization) are now TIER 1 participants in the OMNIA plan.
CentraState and Horizon have a long history of working together to transform the healthcare system into one that promotes health and wellness and provides care focused on value, not volume.
