Leaving the Hospital
CentraState is committed to teaching as part of your care before you go home. A wide range of patient education resources are available to help you prepare for your discharge. Some of your questions may include the following: medications, diet, tests and procedures, activity allowed, and when to call your physician.
Instructions may be given by different members of our healthcare team and may include the nurse, respiratory therapist, social worker, physical or occupational therapist, dietitian and physician.
Let your nurse know if you have questions about your present medical condition or care. She/he will help you coordinate your instructions.
Your physician will write a discharge order on your chart, and either your physician or your nurse will discuss your discharge with you. Checkout time will be determined by your physician. Please make arrangements to leave your room as early as possible. If you have been notified of responsibility for payment not covered by insurance, please stop by the Cashiers Office to make payment or to discuss financial arrangements with a Credit Representative.
Homecare & Private Duty Nursing
Home Health Care
Your Case Manager or Social Worker can assist you and your family in discharge planning and referral to appropriate resources for home care. For information, contact the Quality Improvement/Outcomes Management Department at 294-2835 or the Social Services office at 294-2658.
Health Awareness Assistance
The Health Awareness Center in Freehold offers over 200 health education programs and support groups designed to help you attain your optimal health after discharge. Programs include: CPR, Weight Management, Parent/Baby Classes, and Fitness Classes. Call (732) 308-0570.
The Health Awareness Center is located in the Star and Barry Tobias Ambulatory Campus adjacent to the hospital at 901 West Main Street in Freehold. The Health Awareness Center offers physician lectures, health screenings, fitness classes, and children’s programs. There are also educational books and tapes available. Call (732) 294-0011.
Extended Care Facilities
Arrangements for different levels of care (such as acute rehabilitation, subacute, skilled, assisted living and long-term care) can be made through your Case Manager or through the Social Services Office. Dial Ext. 2658 or 2835.
