PATIENT REPRESENTATIVES

Patient Representatives work to meet the needs of patients and families and enhance patient satisfaction by providing a variety of services, including:

  • Information about hospital services
  • Support during times of crisis
  • Information about patient rights and responsibilities
  • Explanation of hospital policies
  • Assistance with patient issues and concerns

CARE COORDINATION

CentraState’s care coordinators are certified nurses who support patients, their families, and other caregivers by coordinating and monitoring care from pre-admission to post-discharge from the hospital. This may mean helping to arrange a visiting nurse, home health aide, or private nursing services or securing home care equipment – such as a wheelchair, walker, hospital bed, home oxygen or IV therapy. They can also help coordinate home hospice services when needed.

Our care coordinators will work with you or your loved one’s health care team and insurance company, within Medicare/Medicaid guidelines when appropriate, to develop a plan that maximizes comfort and wellness.

SOCIAL WORK

Social workers are specially-trained professionals who help patients and their families meet the social, economical, emotional, and environmental needs they often face. At CentraState, our social workers meet privately with patients and families to discuss their needs.

HEALTH EDUCATION

Your care does not end once you leave the hospital, rather it has just begun. The Star and Barry Tobias Health Awareness Center offers health education programs and support groups designed to help you attain your optimal health. Programs include weight management, stress management, health screenings, and more.

Observation Unit

The Observation Unit at CentraState Medical Center is a designated area within the hospital that allows our physicians to monitor and evaluate the medical condition of patients who do not meet criteria for inpatient admission but are not well enough to go home without requiring further observation. The unit is a part of CentraState’s ongoing effort to improve patient experience by shortening their stay in the Emergency Department and centralizing care for patients.

While under observation, patients will be regularly assessed by a multidisciplinary care team of physicians, nurse practitioners, and other clinical professionals. After providing all necessary diagnostic tests, medications, and therapies, the physician will discuss with the patient whether the patient should be admitted to the hospital as an inpatient or be discharged with follow up services. This observation period generally lasts 24 hours.

FAQs

Yes. For the comfort and convenience of our patients, cell phone use is permitted; however, please refrain from using cell phones in patient rooms, waiting areas, elevators, or other areas that might disrupt patients.

WiFi is available at CentraState Medical Center in certain areas throughout the hospital. To access it, simply turn your WiFi on and connect to the available networks for your device.

Telephone service includes unlimited calling to the following area codes:

  • New Jersey – 201, 609, 732, 848, 856, 908, 973, 551, 862
  • Philadelphia – 215, 267
  • Staten Island, Brooklyn, Queens – 347, 718, 917, 929

To make a call in any of these area codes, dial 9-1-area code-XXXX.

For all other calling areas, dial 8-0-1-area code-XXXX. These calls must be charged to your home telephone or credit card, or you can call collect.

TDDs and portable amplifiers are available for the hearing impaired. For the sight impaired, large button/Braille telephones are also available.

Yes. For your convenience and entertainment, a remote-controlled television is provided in each room. Service includes over 80 Cablevision channels, including several HD channels, along with education programs and information about CentraState services. Tune in to channel 14 to view the program guide.

To turn your television on or off, change channels, or adjust the sound level, use the nurse call pillow speaker that is located at your bedside, located in the side rails of your bed. Your TV/telephone representative can assist you in using this device. Closed caption devices are available for the hearing impaired.

CentraState Medical Center cannot be responsible for any damaged, lost, or missing property. We urge you to leave all valuables and money at home, except for a small amount of cash or a check for newspapers and other sundries. If necessary, the staff can store valuables in the medical center vault.

When you are not using your eyeglasses and dentures, please keep them in a protective case. A denture cup can be provided by the staff on your patient unit. To prevent loss, do not leave dentures on your meal tray or wrapped in tissue.

For the safety of all patients and visitors, we are currently not offering valet parking.

Checkout time will be determined by your physician. Please make arrangements for transportation so that you may leave in a timely manner.
In planning for your return home from the hospital, you may have questions about medications, nutrition, follow-up tests or appointments, activity allowed, and when to call your physician.

Discharge instructions may be given by different members of your health care team, including your nurse, respiratory therapist, social worker, physical or occupational therapist, dietitian, and physician.

Our nurses can help coordinate your instructions, so let your nurse know if you have questions about your medical condition or care. You can also find discharge instructions by accessing our patient portal.

If you have been notified of responsibility for payment not covered by insurance, please stop by the Cashier’s Office located ????????? to make payment or to discuss financial arrangements with a credit representative.

Your care coordinator or social worker can assist you and your family in discharge planning and referral to appropriate resources for home care. Home health care provides assistance to those needing medical care outside of the hospital setting. Some examples of when home health care is useful include:

• Elderly family members who need assistance to remain in their homes
• Patients who need skilled care to recuperate at home after an illness, injury, or surgery
• Families welcoming a new child

Arrangements for various levels of care—such as acute rehabilitation, sub acute care, skilled nursing care, assisted living facilities, and long-term care—can be made through your care coordinator or social worker. Dial ext. 2658 or 2835 from within the hospital for more information.

Hospice care helps improve the quality of life for a person with a terminal illness who chooses to remain home with the support of family, friends, and caring professionals. Hospice care emphasizes comfort measures and counseling to provide social, spiritual, and physical support to terminally ill patients and their families.

All hospice care is under professional medical supervision. The majority of care is provided in the home. Hospice providers have the skills and resources to help a person live as full a life as possible while limiting the physical pain arising from a terminal illness.

In addition to focusing on physical comfort, hospice provides social and spiritual support for patients and their families. This may include time-off for the primary caregiver, personal care, nutritional counseling, pastoral counseling, grief counseling, and help with legal and funeral arrangements.
