Where Quality of Care Meets Affordability

CentraState is committed to ensuring that our communities have access to quality health care at a reasonable cost. That’s why we have partnered with QualCare’s Regional HMO Network and seven other health systems throughout New Jersey to form the Community Care Network (CCN). CCN offers employers access to multiple group health plan options that are affordable and flexible while providing outstanding coverage.

Community Care Health Plans

If rising health care costs and reduced plan options are a problem for you and your employees, there is a solution for small and mid-size businesses. The Affiliated Physicians and Employers Health Plan (APEHP) is now offering four Community Care Health Plans (CCHP) administered by QualCare and including the Community Care Network hospital providers.

The APEHP is a New Jersey self-funded MEWA (Multiple Employer Welfare Arrangement) that includes more than 2,000 businesses and 32,000 members. The MEWA allows employer groups to pool their combined enrollment and claims experience together, resulting in reduced costs, reduced risk, and an opportunity to provide your employees with plan benefits and pricing advantages that are typically reserved for large employer groups.

Advantages of Community Care Health Plans

In addition to attractive group pricing, unique health care options, and a level of administrative flexibility typically not found in traditional small and mid-sized group health care coverage plans, the Community Care Health Plans provide participating employers and their employees with these advantages:

  • Can be offered along with any or all of the other Affiliated Physicians and Employers Health Plan MEWA options administered by QualCare, including PPO, Open Access POS, Network Only (HMO), High Deductible, and HSA/HRA compatible alternatives
  • Maintains a population health focus on wellness, chronic disease management, and coordination of care
  • Primary Care Physician (PCP) referrals are not required
  • National Network Access outside New Jersey is included on most APEHP MEWA plans through the Cigna Open Access Plus (OAP) Network
  • Convenient composite rating and flexible administration allowing employers to have the ability to offer plan options with no active enrollment
  • Multiple prescription plans and optional dental plans
  • COBRA administration included on all plans

Each of the Community Care Health plans offers supplemental benefits that provide plan members with additional value, security, and convenience, including:

  • 24/7 MDLive telehealth services/virtual consults
  • 100% coverage for lab services through Quest
  • Wellness Program incentives

Health Plan Eligibility

Community Care Health Plans are specifically designed for small (2-50 eligible employees) and mid-size employers.
APEHP Group Eligibility Requirements

Eligible groups must be members of one of the following:

  • The medical community
  • Independent Practice Association (IPA)
  • APEHP’s eligible associations
  • Employers Association of New Jersey (EANJ)
  • An active member of one of the following Chambers of Commerce (businesses must meet geographic chamber criteria for membership; please inquire with the Chamber directly for membership requirements):
    • Monmouth Regional Chamber of Commerce
    • Howell Chamber of Commerce
    • Jackson Chamber of Commerce
