CentraState’s Hypertension Management Program aims to empower our community to take control of their heart health. This six-week program includes one-on-one health coaching and education to help participants improve self-management and home monitoring skills. Individuals struggling to control their high blood pressure—or those who have been recently diagnosed with hypertension—can enroll for free.
Working with a participant’s primary care team, certified health coaches are there every step of the way providing guidance on weight loss, stress management, dietary changes, and other healthy lifestyle tips to help get blood pressure under control.
